Use of Medical Appliance during School

Students with a documented need for crutches, a wheelchair, cane, or other device that enables mobility at school must present a Medical Appliance Form completed by the parent and the physician/health care provider prior to returning to school. This form must include that the student has been properly fitted and has been instructed on the proper use of the medical appliance by the physician and/or health care provider. The medical appliance will be provided to the student by the parent and/or physician.

Accommodations needed by students with mobility impairments can vary greatly and the school nurse will make recommendations for school use on a case-by-case basis. If it is determined that a student will need a school elevator, the nurse will instruct the student and staff accordingly.

Students will be allowed to return to physical activity, including but not limited to athletics, when the school receives a medical release from the physician/health care provider.
