Protocol for Responding to Students at Risk for Suicide/Self Injury

  1. RESPOND IMMEDIATELY

Report concerns to administrator/counselor/designee immediately or as soon as possible.

Do not leave the student unsupervised.

 

  1. SECURE SAFETY OF STUDENT

Supervise student at all times.

If necessary contact School Psychologist, School Resource Officer, School Nurse, or additional staff members for assistance.

 

  1. ASSESS FOR SUICIDE RISK

School counselor/school psychologist meets with the student at risk for suicide. 

Administrator/designee collaborates with the school counselor/school psychologist to determine level of risk and action plan.

 

  1. SUSPECTED CHILD ABUSE OR NEGLECT

If applicable, follow mandated reporting guidelines.

 

  1. DETERMINE APPROPRIATE ACTION PLAN

Determine action plan based on level of risk.

If student is transported to hospital, designated staff should accompany student (if parent is unavailable).

Communicate with parent/guardian (courtesy notification to parent/guardian for students age 18 or older with student knowledge).

 

  1. DETERMINE APPROPRIATE FOLLOW UP PLAN

Develop safety plan and include the school psychologist.

Mobilize a support system and provide resources.

Monitor and manage.

 

  1. STUDENT RE-ENTRY GUIDELANES

Develop re-entry plan.

If student transfers to new school, coordinate re-entry with that school.

 

  1. DOCUMENT ALL ACTIONS
